Chronic Care in Singapore
Chronic diseases, coupled with the growing aging population in Singapore, presents a significant
cause for illness and death in modern times.
Common chronic problems such as diabetes, hypertension, high cholesterol and stroke
plague about 1 million Singaporeans. These conditions frequently culminate in
serious complications when not managed properly.
Often, patients seek treatment only when complications surface and this results in unnecessary
suffering of pain and high medical costs. With early detection and good management of these
chronic conditions, these complications can be controlled and even avoided.
Ministry of Health has recognized that disease management programmes can be managed at the
outpatient level and allows MediSave to help pay for part of the outpatient cost - reducing the
direct out of pocket cost for patients.
Some of these diseases include:
- Diabetes
- Hypertension, Lipid Disorders (e.g. High Cholesterol) and Stroke
- Asthma and Chronic Obstructive Pulmonary Disease
- Major Depression and Schizophrenia
- Dementia and Bipolar Disorder
- Osteoarthritis, Benign Prostatic Hyperplasia, Anxiety, Parkinson’s Disease and Nephrosis/Nephritis
